Rising healthcare costs shouldn’t keep you from the treatments you need, that’s why we created our Smile Club Plan, an in-house membership program designed to make dentistry affordable for individuals and families.
This plan provides comprehensive preventive care while lowering the cost of additional treatments. We never compromise on materials, technology, or personalized attention, our patients receive the same exceptional care we’ve delivered to the Orange Park community for more than 30 years.
Preventive care is at the heart of the Smile Club Plan, because catching small issues early keeps your smile healthy for life. And when restorative care is needed, you’ll receive discounted rates that make treatment affordable and worry-free.
Smile Club Membership Options

Membership Details
- Annual fee is due at the time of enrollment
- Plan is effective for one year from the purchase date
- Renewals receive a 10% discount
- Plans not renewed require a $100 reinstatement fee
Individual Plan - 299/year per person
- All exams and digital X-rays
- 2 cleanings with fluoride varnish per year
- 2 prophylaxis or periodontal maintenance appointments
- 20% discount on all other treatments
Family Plan - 549/year for 2 family members
279/year for each additional household member
- All exams and digital X-rays
- 2 cleanings with fluoride varnish per year (per member)
- 2 prophylaxis or periodontal maintenance appointments (per member)
- 20% discount on all other treatments
